Determine Eligibility
Contact the appropriate CDDO for the county where services are being sought. The CDDO will help determine whether you or your loved one qualifies for developmental disability services and assist with case management.
Meet With the CDDO
Schedule a meeting with the CDDO to discuss eligibility and the services available to you. This is an important step in understanding your options and determining the appropriate support.
Visit Dream Vision
Once you are eligible, you can visit or tour one of our homes. A visit gives you an opportunity to learn more about our environment, services and requirements.
Determine the Right Fit
After your visit, you can make a decision based on your experience and whether Dream Vision is a good fit for you or your family member.
Begin Services
Services will begin once all applicable eligibility requirements have been met.
